Job Description
Job Overview We are seeking a compassionate and skilled Visiting Nurse to provide high-quality healthcare services to patients in their homes and manage Clinical Department. The ideal candidate will possess a strong clinical background and the ability to deliver personalized care while ensuring patient comfort and safety. This role requires excellent communication skills, attention to detail, and a commitment to improving patient outcomes. Responsibilities Conduct comprehensive assessments of patients' health status and needs in their home environment. Develop and implement individualized care plans based on patient assessments and medical history. Administer medications, perform treatments, and provide wound care as required. Assist with discharge planning to ensure smooth transitions from hospital to home care. Maintain accurate medical records and documentation of patient care activities. Educate patients and their families on health management, medication administration, and lifestyle modifications. Collaborate with interdisciplinary teams to coordinate care and address any urgent care needs. Ensure compliance with workers' compensation laws and other relevant regulations. Skills Proficiency in Basic Life Support (BLS) techniques. Experience with suturing and Botulinum toxin treatment is a plus. Knowledge of discharge planning processes and MDS (Minimum Data Set) assessments. Ability to ventilate patients as needed in urgent care situations. Strong organizational skills for maintaining medical records accurately. Familiarity with medical office procedures and protocols. Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.
