Clinical expertise. Strong leadership. Real business sense. We need all three. GIA Homecare Services is looking for a Program Director to lead our Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) programs in Massachusetts — and we're looking for someone who can operate at the intersection of clinical excellence and sound business leadership. This isn't a role for someone who's only comfortable in one lane.
CLINICAL EXPERTISE THAT SETS THE STANDARD
You'll be the clinical authority for the program — the person whose judgment shapes how care actually gets delivered:
  • Provide clinical leadership across nursing services, care management, and caregiver training
  • Oversee assessments, evaluations, and plans of care to ensure every member gets safe, effective, person-centered treatment
  • Guide the team through complex clinical cases and changes in member condition
  • Ensure nursing and caregiver services meet every clinical and MassHealth regulatory standard
  • Lead quality improvement initiatives grounded in real clinical outcomes and incident data
LEADERSHIP THAT HOLDS THE WHOLE PROGRAM TOGETHER
Clinical knowledge alone doesn't run a program — leadership does:
  • Lead multidisciplinary team (MDT) meetings and drive collaborative, coordinated care planning
  • Supervise and develop nurses, care managers, and caregivers — with real coaching, not just oversight
  • Make the final call on complex cases, admissions, and clinical escalations
  • Represent GIA with PCPs, VNAs, DDS, DMH, and other outside partners
BUSINESS ACUMEN THAT KEEPS IT ALL SUSTAINABLE
Great care only happens when the operational foundation is solid. You'll bring:
  • Sound judgment on staffing, budget, and resource allocation
  • Comfort tracking and acting on performance metrics — compliance rates, documentation accuracy, staff retention, member satisfaction
  • A compliance-first mindset — full accountability for MassHealth regulations, audit-readiness, and policy adherence
  • The ability to translate clinical priorities into operational decisions that keep the program financially and regulatorily sound
WHAT YOU BRING
  • Bachelor's degree in Nursing (BSN) or healthcare-related field required; Master's preferred
  • Active RN license in Massachusetts
  • Significant leadership experience in AFC, GAFC, or a similar MassHealth-funded program
  • Proven track record combining clinical oversight with operational/business decision-making
  • Valid MA driver's license and reliable transportation WHY GIA?
