Are you ready to practice at the top of your license and bring care to patients where they need to be served? Are your clinical skills fresh in order to be able to start IVs, draw blood and vaccinate? Green Spring Internal Medicine is an award-winning, established internal medicine practice in Lutherville, Maryland. Our mission is to be a team-based practice that places patients at the center of their own healthcare, connects with family and caregivers, and uses health information technology to facilitate seamless transitions between healthcare settings. With 10 years of experience in team-based care management, we are an ideal work setting for a nurse who is motivated to put patient care at the center. We are seeking to expand our team by hiring a nurse, RN or LPN (20- 32 hours/week), a details-oriented and organized individual with experience in care management for adult primary care or geriatric patient populations. Clinical responsibilities to include health risk assessments including Medicare Wellness Visits, the creation of care plans, outreach to high risk patients, care coordination, patient education, and occasional home visits to homebound patients. Skills and ability to independently supervise and assist medical assistants in performing phlebotomy, IV therapy, vaccinations and therapeutic injections. Computer savvy is a must, with the ability to use an electronic medical record and learn by doing. Daily tasks may include telephone outreach to high risk patients, coordination of care with other service providers, networking to provide resources to patients and their caregivers, and working with the team to provide in office service to patients including providing vaccinations and IV therapeutics. Our ideal candidate would be willing to safely conduct periodic home visits to frail elders and patients with disability, with skills and experience in home-based assessment, phlebotomy and simple wound care.
