Nursing Full Job Description RN Nurse Case Manager:
Medical Respite
  • This position requires direct contact with the homeless population of Montgomery County, MD
  • 2 Years RN experience Required
  • 1.
Complete RN assessment, triage, case management, medication monitoring, and patient education to all clients regardless of insurance status. 2. RN will manage all clients requiring medical case management services in the regular shelter beds in coordination with the county RN as clients transition from the medical beds, hospitals, rehabilitation facilities, or the medical respite program. 3. RN will manage clients referred from acute care settings requiring medical case management and/or some level of medical follow-up and make referrals as needed. 4. Collaborate and coordinate client's medical needs/issues with medical respite team. 5. Collaborate on discharge planning with the medical respite team to promote the safe and timely transition of the clients to the shelter system with adequate medical follow-up. 6. Assist clients in obtaining transportation to medical appointments by providing bus tokens, Smart Trip cards, and/or taxicab vouchers. 7. Meet with the medical respite team on a regular basis to coordinate services. 8. Assist clients needing assistance with managing their medications by filling their medication boxes initially and on a weekly basis, providing medication management and education medications, as needed. 9. Conduct group educational seminars on-site for clients on relevant medical topics such as, but not limited to, Diabetes, Hypertension, HIV, TB, Hepatitis, and overall wellness. 10. Document all contacts in clients electronic medical record within 24 hours.
