RN Care ManagerPosition SummaryWe are seeking a Registered Nurse (RN) Care Manager to coordinate and manage patient care across healthcare services. The RN Care Manager will assess patient needs, develop care plans, coordinate services, and support quality outcomes.

Responsibilities Assess patients' clinical, functional, and care coordination needs. Develop and monitor individualized care plans. Coordinate care between patients, providers, and healthcare services. Monitor patient progress and identify barriers to achieving care goals. Facilitate referrals, appointments, and other needed services. Educate patients and families on treatment plans and available resources. Support transitions of care and discharge planning. Communicate patient needs and changes in condition to the appropriate providers. Maintain accurate and timely care management documentation. Collaborate with multidisciplinary teams to promote safe, coordinated care.

Qualifications Active Registered Nurse (RN) license. Graduate of an accredited nursing program. Previous care management, case management, or clinical nursing experience preferred. Strong assessment, care coordination, and critical-thinking skills. Excellent communication and organizational skills. Ability to manage multiple patients and coordinate complex care needs.
