    Key Accountabilities:
    • + Leads and manages a team of staff to ensure effective coordination of patient care across disciplines.
    + Directs the planning, implementation, and evaluation of quality patient care for SHH patients. + Fosters consistent communication with department leaders and direct reports to support collaboration and alignment of priorities. + Builds visibility and accountability across the organization, strengthening relationships with internal and external stakeholders. + Represents the department on committees and in special projects, contributing to organizational strategy and decision-making. + Provides oversight and guidance to clinical teams, ensuring access to appropriate resources and support in alignment with scope of practice requirements. + Reviews and interprets patient/family health data (physical, psychological, and social factors) to inform clinical team decision-making. + Aligns customer needs with appropriate internal resources, including skilled care, personal care, alert follow-up, and other healthcare services. + Ensures compliance with federal, state, and local regulations; monitors and implements changes as required. + Establishes and maintains efficient systems and communication channels to support continuity and quality of care. + Evaluates agency operations and systems, identifying opportunities to improve workflow, productivity, and resource allocation. + Performs other duties/projects as assigned.
    • What is Required?
    • + Graduate from an accredited RN program.
    + 2+ years experience within a home health setting is required, field experience preferred + Prior management experience is required. + Proficiency with MS Office Suite (Word, Excel, Outlook, etc.) + Demonstrated knowledge of the healthcare industry and telehealth. + Experience in surgical nursing, critical care, and experience with Homecare Homebase EMR are preferred. + Must have current drivers license, reliable transportation and insurance in accordance with the laws of the state. + Must be able to sit, stand, bend, lift and move intermittently and be able to lift at least 25lbs
