As an RN Clinical Coordinator , you'll play a vital role in bringing that mission to life. You provide nursing assessment, care coordination, education, and ongoing clinical management for an assigned panel of PACE participants. Key Responsibilities Coordinates nursing care and follow-up for an assigned panel of PACE participants. Completes required nursing assessments. Reviews relevant clinical information, diagnostic results, and outside records using nursing judgment and critical thinking. Develops, reviews, and updates individualized plans of care. Participates in interdisciplinary care planning and clinical discussions. Coordinates and follows up on provider orders, referrals, diagnostic testing, specialist recommendations, medication changes, and other identified care needs. Supports transitions of care following hospitalization, emergency department visits, skilled nursing stays, and other changes in care setting. Provides participant and caregiver education related to disease management, medications, treatment plans, symptom monitoring, and preventive care. Triage and provide appropriate nursing intervention. Maintains complete, timely, and accurate documentation of nursing care and coordination activities. If selected for the position you will receive a full job description. Skills, Knowledge and Expertise Graduate of an accredited School of Nursing. Must have three (3) years' experience as a practicing nurse in a health-related area. Licensed Registered Nurse in the State of Michigan. Must possess a valid driver's license. Must be CPR certified.
