Job Overview Divine Home Healthcare is seeking a part-time Registered Nurse to serve as the Clinical Nurse Supervisor for our small assisted living home in Brooklyn Park, MN. We are a residential-style assisted living facility with a maximum capacity of 5 residents, so this role is ideal for an RN who is looking for flexible part-time work in a smaller, more personal care setting. The Nurse Supervisor will provide clinical oversight for resident care, medication management, assessments, staff delegation, and service plan updates in accordance with Minnesota 144G assisted living requirements. This position does not require constant on-site nursing coverage, but the RN must be dependable, organized, and available to support the facility when clinical guidance is needed. This is a great fit for an RN who has experience in assisted living, home care, long-term care, waiver services, or a similar setting, and who is comfortable working independently with a small team. Duties Provide clinical oversight for a small assisted living facility in accordance with Minnesota 144G requirements. Complete resident nursing assessments, reassessments, and focused assessments as needed. Review and update resident service plans based on current needs, changes in condition, and reassessment findings. Oversee medication management services, including medication reviews, documentation, and follow-up on medication-related concerns. Provide RN delegation and supervision for unlicensed staff performing delegated nursing tasks. Train and educate staff on resident care needs, medication administration, delegated tasks, infection control, documentation, and other required topics. Be available by phone as needed for staff consultation related to delegated nursing tasks or resident care concerns. Review resident documentation, incident reports, changes in condition, and care concerns as needed. Assist with admissions, discharges, service plan updates, and resident care planning. Communicate with residents, family members, case managers, providers, and facility leadership when needed. Help ensure clinical records are accurate, organized, and survey-ready. Support compliance with assisted living regulations, facility policies, resident rights, and safety expectations. Participate in quality improvement efforts and help identify areas where care or documentation can be improved. Provide guidance to direct care staff in a calm, professional, and supportive manner. Qualifications Current Minnesota Registered Nurse license in good standing. Experience in assisted living, home care, long-term care, waiver services, behavioral health, or a related setting preferred. Knowledge of Minnesota 144G assisted living requirements preferred. Experience with medication management and staff delegation preferred. Ability to complete nursing assessments and service plan updates accurately and on time. Comfortable working in a smaller residential assisted living setting. Must be dependable, responsive, and able to work independently. Strong communication and documentation skills. Ability to interact professionally with residents, staff, families, providers, and case managers. Must be able to maintain resident confidentiality and follow HIPAA requirements. Schedule and Pay:
Part-time position Approximately 6 hours per week, with a potential max of 10 hours in a week, depending on resident needs Flexible schedule Some availability by phone may be needed for staff consultation
Pay:
up to$50.00 per hour Additional hours may be available as needed for assessments, admissions, service plan updates, incidents, medication changes, staff training, or survey-related needs.
