Job Description
Job Overview We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) to step into the role of Charge Nurse, leading nursing teams and ensuring exceptional patient care across diverse healthcare settings. As a vital member of our healthcare team, you will coordinate daily nursing operations, supervise staff, and deliver compassionate, high-quality care to patients. Your expertise in clinical procedures, patient assessment, and interdisciplinary collaboration will drive positive health outcomes and foster a supportive environment for both patients and staff. This role offers an exciting opportunity to utilize your extensive nursing experience while making a meaningful difference in the lives of those we serve. Responsibilities Lead and coordinate nursing staff to ensure efficient delivery of patient care in accordance with facility policies, long-term care regulations, and best practices. Oversee patient assessments, vital signs monitoring, and physical examinations to develop personalized care plans. Administer medications, injections, IV infusions, and perform catheter care following strict protocols and medical documentation standards. Manage complex clinical procedures such as tube feeding, sterile processing, wound suturing, and airway management. Ensure compliance with HIPAA regulations and maintain strict confidentiality of all medical records and electronic health records (EHR/EHR systems). Conduct triage assessments for incoming patients or residents, prioritizing urgent needs such as acute pain management or infectious disease care. Collaborate with interdisciplinary teams including physicians, therapists, case managers, and family members to coordinate discharge planning and post-acute care. Utilize Point Click Care EMR system to document patient care accurately and efficiently. Provide mentorship and supervision to nursing staff while fostering a culture of continuous improvement and professional development. Experience Valid Licensed Practical Nurse (LPN) license with current certification in CPR/BLS. Demonstrated critical care experience in settings such as ICU, emergency room, or trauma centers is highly preferred. Extensive knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and medication administration protocols. Proven experience working with diverse populations including geriatrics, individuals with developmental disabilities, dementia or Alzheimer's disease, and memory care patients. Familiarity with Point Click Care EMR/EHR system; experience managing medical records digitally is advantageous. Prior experience in long-term care facilities such as skilled nursing facilities or assisted living communities is desirable. Strong background in acute pain management, infection control practices, ventilator management, and hospice or palliative care is beneficial. Ability to perform complex clinical procedures including venipuncture, phlebotomy, catheterization, sterile technique, and first aid.
