Job Description
Mercy seeks a part-time RN Care Manager for weekend shifts to coordinate patient care across settings and support safe transitions. In this role, you'll complete comprehensive nursing assessments, develop and adjust care plans, and collaborate closely with physicians, therapists, and social workers. You'll educate patients and families, arrange community resources, and document care in the EHR. Responsibilities Provide weekend care management and coordination for assigned patients across inpatient and outpatient settings Conduct comprehensive nursing assessments and develop individualized care plans Collaborate with physicians, therapists, and social workers to optimize patient outcomes Educate patients and families on treatment plans, medications, and self-care strategies Monitor clinical status, track progress, and adjust care plans based on patient response Facilitate safe transitions of care, including discharge planning and community resource referrals Document all assessments, interventions, and communications in the electronic health record Participate in interdisciplinary team meetings and quality improvement initiatives at Mercy Required Skills Care management Patient assessment Care planning Discharge planning Clinical documentation (EHR) Interdisciplinary collaboration Patient and family education Utilization management Chronic disease management Time management
