The Care Manager facilitates the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. In addition, the Care Manager helps drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements and patient satisfaction). The Care Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of service, promote efficient utilization of resources and ensure a well-organized and safe plan of care is established for every patient. Responsibilities Anticipates the needs of patients and family members related to the emergency room visit and discharge plan. Assesses and determines the most appropriate discharge plan for patients who have been released from IVC or requesting voluntary detox services. Develops and implements plans of care which enhance quality, access, and cost-effective outcomes. Effectively communicates patients and/or family during crisis and traumas. Ensures patient's participation in the treatment planning process. Creatively identifies and links patients to appropriate organizational, financial and community resources in order to achieve optimal outcomes and compliance with regulatory agency requirements. Provides coverage to inpatient units after hours, as assigned.
Minimum Requirements Education:
Associates or Bachelors of Science in Nursing or higher, or Bachelors of Science in Social Work or higher is required. If a Registered Nurse, current licensure as a Registered Nurse by the NC Board of Nursing is required. Pay Range $65520.00 - $95501.12 Other Information Shift explanation: Friday, Saturday, Sunday. 40 hours. 3- 13.33 hour shifts.
