HOME CARE CLINICAL MANAGER - RN
Job Description:
Provide coordination, case management of all patient services. Provide appropriate documentation for both patient care and staff performance. Supervise assigned Care staff. Assist Administrator as necessary. Provide direct patient care as necessary to include case management, Oasis care planning, assigning Care staff, and ensuring the efficient, effective utilization of existing staffing resources.
Job Requirements:
Current RN licensure within state of branch location, and successful completion of nursing education from an approved school of nursing. Five (5) years of clinical nursing experience that encompasses a variety of clinical settings. Three (3) years of home health experience preferred In-depth knowledge of homecare regulations. Ability to independently organize work, provide appropriate case management, cost-effective home healthcare services, and timely, accurate documentation of said care. Strong critical thinking and communication (oral and written) skills and literacy in the English language. Ability to use a personal computer and have a working knowledge of a variety of computer applications (Word, Excel, Window, etc). Minimum of twelve hours of continuing education annually.
