As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and rehabilitation services to our patients and residents.

Responsibilities:

Provide patient care and rehabilitation services under the supervision of a RN or physician, including assessing patient needs, developing care plans, and implementing treatment programs Collaborate with the interdisciplinary team to ensure comprehensive care and achieve optimal patient outcomes Administer medications, treatments, and injections as prescribed by physicians Monitor patient vital signs and report any concerns or changes to the RN or physician Maintain accurate and detailed patient records, including progress notes and treatment plans Participate in the development and implementation of individual patient care plans Continuously assess and evaluate patient care needs and make adjustments as necessary
