We are looking for an experienced Registered Nurse to join our team as an RN Service Supervisor . This is an opportunity to play a meaningful role in shaping the quality of care our clients receive while helping build a growing, relationship-centered home care agency. Job Description As the RN Service Supervisor , you will serve as a key member of Heard Home Care's clinical team, providing nursing oversight for clients and caregivers receiving our in-home services. You will use your clinical expertise and professional judgment to assess client needs, develop individualized Plans of Care, supervise caregiving staff, and ensure services are delivered safely, appropriately, and in accordance with agency policies and North Carolina requirements. This role is ideal for an experienced nurse who is confident working independently, has strong clinical judgment, and understands that exceptional home care requires both clinical excellence and genuine human connection. You will have the opportunity to make a direct impact on clients and their families by ensuring that the right care is provided by the right caregiver. What You'll Do Conduct initial nursing assessments for new clients. Develop individualized Plans of Care based on client needs and assessment findings. Review and update Plans of Care as client needs change. Conduct required 90-day/quarterly supervisory visits and annual supervisory visits in accordance with agency policy and applicable regulations. Monitor clients' general condition, progress, and response to services. Supervise designated in-home aides, companions, sitters, and respite staff. Validate caregiver competencies through observation and return demonstration. Onboard and provide clinical guidance to new caregivers. Ensure caregivers are performing assigned duties in accordance with their job descriptions and the client's Plan of Care. Document assessments, supervisory visits, competency evaluations, and other required clinical activities. Communicate with clients, families, caregivers, healthcare providers, and agency leadership as needed. Identify changes in client condition or care needs and make appropriate recommendations. Assist with caregiver training and continuing education. Assist with staffing, scheduling, caregiver performance evaluations, and other operational needs as appropriate. Promote client satisfaction, safety, dignity, and quality of care. Remain available for clinical supervision and consultation during assigned service hours. Required Current, unrestricted Registered Nurse (RN) license in North Carolina with no adverse findings. At least 1 year of RN supervisory or clinical leadership experience . Valid driver's license. Reliable personal vehicle. Current automobile insurance meeting North Carolina requirements. Reliable transportation and willingness to travel to client residences throughout the agency's service area. Strong clinical assessment, documentation, communication, and organizational skills. Ability to work independently and exercise sound clinical judgment in a home-care environment. Ability to maintain client confidentiality and comply with applicable privacy requirements. Preferred 8+ years of professional bedside nursing experience. Previous experience in home health, home care, private-duty nursing, geriatric care, community nursing, or disability services. Experience conducting comprehensive nursing assessments. Experience developing and implementing Plans of Care. Experience supervising nursing or direct-care staff. Experience conducting caregiver competency assessments or return demonstrations. Experience working with older adults and/or adults with intellectual and developmental disabilities.
