Position Overview:

Eldercare is seeking an experienced and compassionate Registered Nurse (RN) Care Manager to join our dedicated team. The RN Care Manager plays a vital leadership role in ensuring exceptional client care, supporting field staff, and strengthening community partnerships. This position combines clinical oversight, care management, and community outreach, making it ideal for a nurse who thrives on both relationship-building and operational excellence.

Schedule:

Monday-Friday, 9:00 AM - 3:30

PM Position Type:

32 hours per week

Key Responsibilities:

Conduct in-home and facility-based assessments to determine client care needs. Collaborate with clients and families to develop individualized plans of care. Provide ongoing care management and clinical oversight to ensure quality outcomes. Supervise and support field staff through visits, evaluations, and competency checks. Coordinate admissions and ensure effective communication between clients, scheduling, and clinical teams. Manage and resolve client concerns, incidents, and infection control issues. Participate in new staff orientation, training, and evaluations. Build and maintain relationships with hospitals, physicians, rehab centers, and senior communities to generate referrals. Conduct educational presentations and outreach to referral partners and the community. Participate in the RN on-call rotation as needed.

Qualifications:

Active Registered Nurse (RN) license in North Carolina, in good standing. Minimum 2 years of clinical experience (home care experience preferred). Valid driver's license and reliable transportation. Strong communication, leadership, and organizational skills. Professional, compassionate, and able to build trust quickly. Self-motivated and goal-oriented, with the ability to balance field and administrative responsibilities.

Work Environment:

This position is primarily office-based, Monday-Friday, 8:00 AM - 3:30 PM, with field visits and community outreach as required.
