No Department Details As an RN Care Manager in our Clinics, you'll be at the heart of patient care—helping individuals manage their health and supporting them every step of the way. You'll guide patients through their care journey by creating personalized care plans, providing education, and ensuring smooth transitions between clinic visits, specialty care, and hospital services when needed. You'll work side by side with providers and team members to coordinate chronic disease management, medication reconciliation, and preventative health, all while building strong relationships with patients and their families.- Monday through Friday, Days
