Regional travel throughout Nebraska as needed We are seeking an experienced Registered Nurse (RN) for a corporate-level Care Navigation Director opportunity with a highly respected senior living and post-acute healthcare organization in the Omaha area. This position will lead referral management, clinical review, admissions support, and transitions of care across multiple communities throughout Nebraska. The Care Navigation Director will serve as an important clinical liaison between hospitals, case managers, discharge planners, physicians, referral partners, and senior living/post-acute communities . This RN will review clinical referrals, assess prospective admissions, and help determine the most appropriate placement based on each individual's clinical needs and the services available within the organization. This is an excellent opportunity for an RN with experience in skilled nursing, post-acute care, care transitions, admissions, case management, utilization review, nurse navigation, or clinical liaison work who is ready to advance into a multi-site leadership position. Key Responsibilities Lead and coordinate care navigation and referral activities across multiple communities. Review clinical referrals and medical documentation to determine appropriate placement and level of care. Utilize clinical nursing judgment to evaluate prospective admissions. Manage incoming referrals through electronic referral and admissions platforms. Collaborate with hospital case managers, discharge planners, physicians, and other healthcare professionals. Partner with community clinical and operational leadership regarding admission decisions and clinical capabilities. Support admissions teams with referral review, response management, and best practices. Help ensure timely and successful transitions from hospitals and other healthcare settings. Maintain accurate documentation within referral management and EMR systems. Build and maintain strong relationships with hospital systems and other referral partners. Monitor referral trends and performance metrics to identify opportunities for improvement. Balance resident clinical needs with available community resources and capabilities. Travel throughout Nebraska as needed to support communities and referral relationships. Qualifications Active Registered Nurse (RN) license required. Minimum of 3 years of experience in senior care, skilled nursing, post-acute healthcare, care transitions, admissions, case management, or a related clinical setting. Strong clinical assessment and critical-thinking skills. Experience reviewing medical records and clinical documentation. Understanding of post-acute admissions and referral processes. Experience with EMR systems, electronic referral platforms, CRM systems, or similar healthcare technology. Strong communication and relationship-building skills. Ability to communicate effectively with hospital case managers, discharge planners, providers, residents, families, and healthcare leadership. Strong organizational skills and ability to manage multiple priorities. Leadership or multi-site experience preferred. Willingness to travel throughout Nebraska as needed.
