About the Role We're seeking a compassionate and experienced RN Manager to join our leadership team. This is a unique hybrid role that combines clinical field work with supervisory responsibilities, giving you the opportunity to make a difference on every level. You'll lead and support our clinicians while remaining actively engaged with patients in the field—ensuring care is not only coordinated and compliant, but deeply human and dignified. What You'll Do Supervise and mentor home health and hospice nursing staff, providing clinical guidance and support. Review patient cases, documentation, and care plans to ensure high-quality, coordinated care. Participate in case conferences and interdisciplinary team meetings. Provide direct patient care as needed—staying connected to the heart of home health nursing. Lead orientation and training for new team members. Support continuous quality improvement efforts and ensure adherence to CHAP standards. Partner with the Clinical Director to promote a positive, collaborative, and mission-aligned culture.
