Qualifications:
Education, Knowledge, Skills and Abilities Required:
Associate's Degree in Nursing or Diploma Certificate in Nursing. Minimum of two (2) or more years RN Experience. Minimum of three (3) years certified home care experience. Excellent written and verbal communication skills. Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.Education, Knowledge, Skills and Abilities Preferred:
OASIS certified. BSN.Licenses and Certifications Required:
NJ State Professional Registered Nurse License. BLS AHA Basic Health Care Life Support HCP Certification.Responsibilities:
Responsible for assisting the clinical operation by providing telephonic case management as well as quality, patient satisfaction and process flow initiatives and reports to the Manager of Clinical Practice. Collaborates with specialists, other physicians, and professionals at local healthcare facilities and in the community, to provide home health services to patients in need of skilled nursing care. Coordinates care by ensuring effective communication and completion of treatments/procedures. Utilizes the nursing process (assessing, planning & implementing care) to provide nursing visits &/or manage a nursing case manager caseload, as needed. Provides comprehensive care and teaching to patients and families regarding the individualized plan of care. Applies clinical knowledge and skills (i.e., administration of medications, treatments, procedures, etc.) to safely provide direct patient care in a professional, caring, culturally- sensitive, and cost efficient manner; ensuring appropriate clinical coordination and support of patients and families.
