Daughters of Sarah is a 210-bed skilled nursing and rehab facility conveniently located on Washington Ave. Extension in Albany (within close proximity to Crossgates Mall).

Your basic responsibilities will include:

Performing routine nursing evaluations, monitoring vital signs and other clinical indicators, and communicating significant changes in residents' conditions to the RN, physician, or other appropriate healthcare professionals. Assisting with admissions, transfers, discharges, and other nursing protocols while ensuring required documentation is completed accurately and timely. Collaborating with physicians, registered nurses, therapists, nursing assistants, and other members of the interdisciplinary team to support continuity and quality of resident care. Supervising and providing guidance to Certified Nursing Assistants (CNAs) and other assigned nursing staff to ensure resident care, hygiene, safety, and daily needs are appropriately addressed. Delegating and overseeing appropriate nursing and resident-care tasks within the scope of practice of assigned staff. Monitoring residents' activities of daily living, including personal hygiene, nutrition, hydration, mobility, and other needs identified in the resident's care plan. Maintaining accurate and timely nursing documentation, including progress notes, medication and treatment records, care plan updates, incident reports, and other required clinical records. Reviewing resident care and reporting concerns, changes in condition, incidents, or unusual occurrences to the RN, Director of Nursing, physician, and/or responsible party as appropriate. Providing direction, education, and coaching to nursing assistants and other assigned staff regarding proper resident-care techniques, infection prevention, safety, and facility standards. Assisting with the orientation and training of new nursing staff and providing ongoing education or re-training when needed. Following infection control procedures, safety protocols, medication administration standards, and all applicable facility policies and regulatory requirements. Participating in investigations and follow-up related to incidents, accidents, medication errors, changes in resident condition, or other unusual occurrences. Maintaining effective communication with residents, families, healthcare providers, and members of the interdisciplinary team to promote resident safety, dignity, and quality of life. Providing direct nursing care to residents in accordance with their individual care plans, facility policies, and applicable state and federal regulations. Administering medications and treatments as ordered, monitoring residents for responses or adverse reactions, and promptly reporting changes in condition to the appropriate healthcare provider. Performing other nursing and supervisory duties within the LPN's scope of practice as assigned by the RN, Director of Nursing, or facility leadership.

Requirements:

A current New York State LPN license. A minimum of one (1) year of clinical experience. 1 year of experience in long-term care and leadership/ management experience preferred.
