$140,000 - $150,000 per year Job Details - Responsible for overseeing the hospital's Case Management department and leading RN case managers in coordinating high-quality, cost-effective patient care. Manages care transitions, utilization review, discharge planning, and collaboration with physicians, nurses, social workers, insurance providers, and post-acute care facilities. Ensures appropriate resource utilization, regulatory compliance, patient advocacy, and timely discharge planning while promoting optimal clinical and financial outcomes.
