Qualifications:

Must be a graduate of an accredited school of practical nursing and a current licensure with the New York State Board of Nursing Excellent Communication Skills Experience in long term care preferred Must be willing to work 1 weekend a month.

Job Duties:

• Adheres to service excellence service excellence by developing and maintaining positive, respectful relationships with all customers, internal and external, to include: elders, families, team members, all staff throughout the organization and community partners.
• Conducts comprehensive chart audits.
• Communicate with Rehab managers and ADON daily as an opportunity to be aware of Complaints, Accident/incidents, resident status, and general organization operations, daily tasks needed to be completed.
• Actively participates in the committees as requested.
• Assist in the collection of information for Utilization Review, including completion of audits as assigned.
• Acts as a backup in the absence of a Nurse Manager.
• Monitors Sigma Care dashboard for completeness. Includes Assessments/MDS being completed on time, CNA documentation completed, Documents reviewed, CNA instructions completed, LPN tasks completed. Also ensures Care plan's are updated and match CNA instructions in conjunction with RN Managers.
• Assist with the completion of admission and discharge duties.
• Schedule and monitor laboratory results.
• Participates in care-planning with the rehab team to ensure positive outcomes and meet target discharge dates.
• Ensures required Medicare charting completed on a daily basis for each resident by a Registered Nurse.
• Assumes the role of the medication/treatment nurse on the med cart, in the absence of the LPN, administering medications/treatments per policy/procedure as needed.
• Assist with performing Quality Assurance audits as directed by

ADON/DON.

• Responsible for the monitoring and possession of the supervisor phone until evening supervision assumes this responsibility.
• Other duties as assigned.
