A leader in acute inpatient rehabilitation on Long Island, St. Charles Campus provides specialized care for patients recovering from conditions such as stroke, traumatic brain injury, spinal cord injury, and orthopedic injuries. Job Details Our Care Manager (RN) will: Assess, plan, implement, monitor and evaluate options and services to effect an appropriate individualized plan of care for patients across the acute care continuum. Perform required activities for utilization management, PRI assessment and completion, transition planning, and identification of actual or potential quality/risk indicators, in compliance with regulations and organizational goals. Perform as an integral part of the interdisciplinary team to effectively ensure optimal patient outcomes and length of stay efficiency. Collaborate and coordinate with multidisciplinary teams to facilitate a safe quality experience for patients though the continuum of care. Be required to pursue ongoing education, certification and self-development to remain current with industry standards and business objectives related to Care Management as appropriate. Have knowledge of The Joint Commission standards, and Federal, State and PRO regulations preferred. Advocate for and build meaningful relationships with patients and their loved ones through safety intervention, emotional support, and education. What you'll bring: Passion for Care Management 3+ Years acute care clinical RN experience required Prior RN experience in Utilization Management, Performance Improvement, Transition Planning and/or Care Management experience in an acute hospital or related managed care setting preferred
