Job Description

LPN Supervisor Job Description Job Summary The LPN Supervisor assists the Director of Nursing (DON) in the oversight and management of the nursing and HHA staff in providing resident services and care in accordance with the New York State Department of Health regulations and the mission of Judson Meadows. This position provides direct supervision to Licensed Practical Nurses and Home Health Aides and may include med cart responsibilities. This role requires a strong understanding of patient care practices and the ability to foster a collaborative environment among staff. Responsibilities Personnel and Scheduling Assist the DON with creating and maintaining the nursing staff schedule Assist with orientation, training, and supervision of HHAs and nursing staff Participate in staff meetings and in-service education Assist DON with corrective actions, performance reviews, and disciplinary measures as needed Promote interdepartmental teamwork and customer satisfaction Resident Care Responsibilities Provide direct care when needed Answer call bells and respond to resident needs promptly Ensure staff are following resident care plans Administer medications and monitor medication supplies Complete medication self-management audits Observe and report physical, cognitive, or emotional changes in residents Update MARS, task sheets, ISPs, and functional assessments Maintain accurate and up-to-date resident files Communicate with residents and families professionally and compassionately Follow all elopement and emergency response procedures Recordkeeping & Compliance Maintain resident records per NYS regulations Record and report incidents, accidents, and care notes Ensure controlled substances are checked in and logged properly Assist in quality assurance programs and policy development Participate in audits and follow-up interventions Monitor and order nursing, resident care, and office supplies as needed E. Safety and Infection Control Report and act on safety hazards promptly Follow all universal precautions and infection control procedures Participate in fire, disaster, and emergency preparedness drills Know individual roles during emergencies and evacuations 60% paperwork functions 20% resident care 10% staff supervision 10% other managerial duties Knowledge of

regulations Case management duties as assigned by DON Supervision and delegation of duties to HHAs and LPNs Transcribe physician orders and maintain/update MARS Administer treatments and wound care using proper infection control Accurate documentation of care provided Qualifications A. Educational Requirements Graduate of an accredited School of Nursing Experience Minimum of 2 years of nursing experience with elderly populations preferred Supervisory experience strongly preferred Specific Requirements Ability to work flexible hours, including some weekends/holidays as needed Effective verbal and written communication skills in English Ability to work calmly and effectively in emergency or emotionally charged situations Strong interpersonal skills and professionalism Organizational and documentation skills Demonstrates leadership, patience, and adaptability D. Physical and Sensory Requirements Ability to move intermittently throughout the workday Ability to lift, push, pull, and carry up to 80 lbs Emotional and mental resilience Ability to hear, see, and communicate clearly Ability to assist in resident evacuations if necessary Current registration and LPN license in good standing with the NY State Board of Nursing Ability to work collaboratively with multidisciplinary teams to provide optimal patient outcomes.
