Requirements:

BSN required Active NYS RN License required PRI Certification required within 3 months of employment Screen Certification strongly preferred Care Management Certification preferred (CCM, CMC, etc.) Minimum 3 years of clinical experience required ED, Critical Care, or Med/Surg experience preferred Previous Case Management experience preferred Experience with Milliman/InterQual criteria preferred Knowledge of healthcare financial, regulatory, payer, state, local, and federal programs preferred Strong computer skills required

Key Responsibilities:

Perform comprehensive patient assessments upon admission Coordinate multidisciplinary patient care and discharge planning Facilitate patient/caregiver education and aftercare arrangements Conduct utilization review activities and payer reviews Collaborate with healthcare teams to ensure safe discharge planning Support readmission prevention, LOS reduction, and patient satisfaction initiatives Maintain accurate documentation and compliance with department policies #perm2026
