(RN) - IMMEDIATE OPENING Make a difference. Lead with purpose. Central Star Home Health is seeking an experienced Registered Nurse Supervisor to join our team. If you have home health/home care experience and enjoy providing clinical leadership while making a difference in the lives of patients and families, we want to hear from you!

Assess patients and develop individualized plans of care. Conduct home visits and monitor patient progress. Provide clinical supervision and support to caregivers and nursing staff. Ensure appropriate caregiver assignments based on patient needs and physician orders. Serve as a liaison between patients, families, caregivers, physicians, and the branch office. Ensure safe, high-quality patient care and a safe home environment. Complete required clinical and supervisory documentation. Provide staff orientation, education, and ongoing training.

Qualifications:

Current RN license 3+ years of nursing experience as an RN Home Health/Home Care experience strongly preferred Previous supervisory experience preferred Strong communication, assessment, and organizational skills Reliable transportation for patient visits
