8:00 am - 4:30 pm, days are aligned with business needs, occasional weekends Your role with us As a registered nurse care manager at Salem Health, you play a central role in coordinating patient care from admission through discharge, beginning with early assessment to identify high-risk needs, guide utilization and initiate discharge planning. They collaborate closely with patients, families, and interdisciplinary teams to manage care plans, address barriers and ensure smooth transitions to appropriate post-acute services while preventing readmissions. The role also involves overseeing resource utilization, educating patients and staff, and leading care coordination efforts to promote safe, efficient and patient-centered outcomes. What you'll do Conduct patient assessments within 24 hours to identify high-risk cases, utilization needs, and discharge planning priorities, including appropriate referrals to Social Work. Lead care coordination and discharge planning for assigned patients, ensuring safe, efficient transitions across the continuum of care. Collaborate with patients, families, physicians, and interdisciplinary teams to develop and implement individualized care plans. Use clinical judgment to manage complex and high-risk cases, proactively addressing barriers, reducing readmissions, and improving care outcomes. Oversee resource utilization and patient status such as inpatient vs. observation, ensuring compliance with regulatory requirements and payer guidelines. Facilitate appropriate post-acute care placement and services, aligning patient needs, preferences, and available resources. Participate in care coordination rounds, quality improvement efforts, and staff education to enhance care management practices. Required qualifications Associate degree of Nursing (ADN), ADN with five years of experience working of acute care RN experience or care manager, or ADN with an agreement to complete Bachelors of Science in Nursing (BSN) or Masters of science in Nursing (MSN) within 2 years of hire required. BSN or BAN or MSN or non nursing bachelors degree with MSN may be accepted in lieu of ADN. Active Basic Life Support Certification required. Current unencumbered licensure to practice nursing as a Registered Nurse in State or Oregon required. One year experience required. Additional information Care management experience preferred.
