Care Manager (RN) Full-Time | Monday-Friday | No Weekends or Holidays Internal Medicine The Care Manager provides patient-centered care coordination, helping patients navigate the healthcare continuum while improving health outcomes and patient experience. This role focuses on chronic disease management, care planning, care transitions, closing care gaps, and connecting patients with community resources. What You'll Do Coordinate care plans and support patient care transitions Educate patients and families to support self-management Identify care gaps and barriers to care Connect patients with community resources and support services Collaborate with providers, care teams, and community partners Skills & Qualifications Strong communication, relationship-building, and critical thinking skills Ability to work independently and collaboratively Strong organizational and multitasking abilities Compassionate, patient-focused approach to care Experience with EHR systems and Microsoft Office programs Education & Licensure Associate degree in Nursing required; Bachelor's degree preferred Current RN license and CPR certification required Clinical nursing experience required; care management experience preferred Keystone Health is an Equal Opportunity Employer
