Job Description

Could you be our next Care Team Coordinator at HomeCare & Hospice? Why work as a Care Team Coordinator with Main Line Health? Make an Impact! The Care Team Coordinator (CTC) is responsible for the daily operations of the patient care team within HomeCare & Hospice. Individual is responsible for the coordination and scheduling of all clinical care across the disciplines and health care providers. The CTC coordinates the day, evening and weekend operations of the care team. The Care Team Coordinator works in conjunction with the Division Manager to develop and monitor patient care services based on customer needs. The Care Team Coordinator is responsible for clinical outcomes and patient satisfaction for designated team(s). The CTC provides leadership within the clinical team and facilitates the provision of high-quality patient care and services. They participate in performance improvement initiatives of the organization. The Care Team Coordinator oversees new employee orientation, on-going staff competencies, and assists with 90 day and annual performance reviews. The CTC is accountable for employee engagement for designated team(s). Develop and Grow Your Career! Position:
Care Team Coordinator -
Radnor Shift:
Day shift (8:30 am to 5:00 pm); Monday -
Friday Experience:
1. Five years clinical experience required. 2. Three years' experience in the home care or hospice setting preferred.
Education:
Registered Nurse, BSN required. MSN or equivalent preferred.
Licensures/Certifications:
1. Licensed in Commonwealth of PA OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact. 2. CPR Certification 3. Current Driver's license and proof of auto insurance required. 4. Specialty nursing certification preferred.
