Position Overview The Care Manager Team Lead provides day-to-day leadership and workflow support for the Care Managers assigned to the clinic(s). This working lead position helps ensure that patient care activities, documentation, outreach, care-plan tasks, and clinic communications are completed accurately, consistently, and within established timelines during Central Time operating hours. The Team Lead serves as the first point of contact for routine team questions and escalations, monitors team performance and workload, and partners with Care Management leadership to maintain a high-quality experience for patients and the clinic. The position also maintains an assigned patient workload or provides direct care-management coverage based on operational needs.
Key Responsibilities Team Leadership :
Provide daily guidance, coaching, and workflow support to Care Managers assigned to the clinic(s).
Workflow Oversight :
Monitor daily queues, patient assignments, clinical messages, care-plan tasks, outreach activity, and outstanding documentation to ensure timely completion.
Clinic Coordination :
Serve as a primary operational contact for clinic(s) needs and maintain clear communication between the clinic, Care Managers, and internal leadership.
Patient Care Support :
Maintain an assigned patient workload or provide coverage as needed, including patient outreach, education, care coordination, and follow-up within the applicable care management programs.
Escalation Management :
Respond to routine patient, clinic, and team concerns; escalate clinical, compliance, staffing, KPIS, or service issues to the appropriate leader when necessary.
Quality Assurance :
Review documentation, care-plan activity, patient communications, and workflow completion for accuracy, timeliness, and adherence to company and clinic standards.
Performance Support :
Track productivity, quality, responsiveness, and other established performance expectations; provide timely feedback and identify coaching or training needs.
Workload and Coverage :
Assist with daily assignments, workload balancing, schedule coverage, and contingency planning to support uninterrupted operations.
Training and Onboarding :
Support the onboarding, training, and continued development of new and existing Care Managers assigned to the clinic(s).
Process Improvement :
Identify workflow barriers, recurring issues, and opportunities to improve efficiency, communication, documentation quality, and the patient experience.
Compliance :
Protect patient information and ensure team practices align with HIPAA, Prevounce policies, clinic requirements, and applicable healthcare standards.
Additional Duties :
Perform other related duties as assigned based on organizational and operational needs. Required Qualifications At least two years of experience in care management, chronic care coordination, remote patient support, ambulatory care, or a related clinical environment. Previous experience serving as a team lead, preceptor, trainer, or other informal or formal leadership resource. Proficiency with electronic medical records, clinical messaging portals, task-management systems, and standard office software. Strong clinical judgment and the ability to recognize concerns that require escalation to a provider or clinical leader. Excellent written and verbal communication skills, including the ability to communicate professionally with patients, clinic staff, providers, and employees. Strong organization, prioritization, problem-solving, and follow through in a fast-paced remote environment. Ability to work Monday through Friday during business hours and maintain a private workspace with reliable high-speed internet. Preferred Qualifications Experience with Remote Patient Monitoring, Chronic Care Management, Advanced Primary Care Management, or other value-based care programs. Previous experience leading a remote clinical or care-management team. Experience monitoring team metrics, conducting documentation audits, and coaching employees on productivity and quality expectations. Experience supporting a high-volume physician practice or multiple provider workflows.
