Position Summary We are seeking an experienced and dependable Registered Nurse (RN) to serve as a Part-Time Clinical Manager for our growing home health agency. This hybrid role combines clinical oversight, documentation review, regulatory compliance, and patient assessments. The RN must be available by phone during operating hours and able to complete Start of Care (SOC) assessments within 48 hours of receiving a referral. Key Responsibilities Oversee clinical operations and staff compliance Complete SOC assessments and in-person patient evaluations within 48 hours of referral Review OASIS assessments, Plans of Care, and EHR documentation Provide phone support for clinical questions and care coordination Communicate with physicians, staff, patients, and families as needed Ensure compliance with applicable state, Medicare/Medicaid, and agency requirements Review charts for accuracy, medical necessity, completeness, and timeliness Qualifications Active, unencumbered RN license in [State] 1-2 years of home health experience preferred Knowledge of
