Job Description

Job Summary Trusted Path is a nurse-led geriatric care management company seeking a compassionate and experienced Registered Nurse (RN) to join our growing team. Our Care Management Nurses work one-on-one with older adults and their families, helping them navigate healthcare, aging, chronic illness, dementia, Parkinson's disease, and changes in care needs. This is a great opportunity for an experienced nurse looking for meaningful nursing work with flexibility and autonomy outside of the traditional bedside setting. Flexible Schedule One of the biggest benefits of this position is flexibility. There are no traditional set shifts or office hours. You will have the ability to: Work from home for documentation, phone calls, and care coordination Schedule client visits on days and times that work with your personal schedule and the client's needs Independently organize your schedule and caseload Maintain greater flexibility and work-life balance than a traditional nursing schedule This position will begin part-time with the potential to transition into a full-time role as our client base continues to grow. Responsibilities Complete comprehensive nursing and geriatric assessments Develop and update individualized care plans Coordinate care with physicians, specialists, therapists, home health agencies, caregivers, pharmacies, and other providers Accompany clients to medical appointments when needed Provide medication management and health monitoring Educate clients and families about medical conditions and changes in care needs Support individuals and families navigating dementia, Parkinson's disease, and other age-related conditions Identify fall risks and home safety concerns Help families locate and coordinate community resources and services Advocate for clients throughout the healthcare system Communicate with family members and healthcare providers Complete timely documentation and ongoing care coordination
Qualifications Required:
Active, unrestricted Maryland RN license Minimum of 3 years of nursing experience Strong clinical assessment and critical-thinking skills Excellent communication and organizational skills Ability to work independently and manage your own schedule Comfortable working with older adults and families Reliable transportation for local client visits Comfortable with technology and electronic documentation
Preferred:
Geriatric nursing experience Case management or care management experience Home health, neurology, psychiatry, hospice, rehabilitation, or community nursing experience Experience caring for individuals with dementia or Parkinson's disease Gerontological nursing or case management certification Compensation & Benefits $42-$50 per hour , based on experience Flexible part-time schedule Potential to grow into a full-time position Remote work when not seeing clients No traditional set shifts Significant autonomy in scheduling client visits Meaningful one-on-one relationships with clients and families About Trusted Path Trusted Path is a nurse-led geriatric care management company helping older adults and their families navigate the complexities of aging and healthcare. Pay:
$42.00 - $50.00 per hour Expected hours: No less than 20.0 per week
