Clinical Coordinator- Registered Nurse Facility location: White Oak Manor York - 111 S Congress Street, York, SC 29745 Are you a compassionate and driven Registered Nurse looking to make a meaningful impact? We are seeking a flexible, dependable, and quality-focused RN Clinical Coordinator We're looking for a We want YOU to join our exceptional team as an RN Clinical Coordinator to join our team who's flexible, dependable, and passionate about quality care. You'll assess resident needs, support care plan implementation, and provide compassionate clinical care. Lead and mentor CNAs while helping the team deliver consistent, person-centered care every day. In this leadership role, you will assess resident needs, support and oversee care plan implementation, and provide hands-on, compassionate clinical care. You will play a key part in ensuring consistent, person-centered care while serving as a mentor and resource to our CNAs and nursing staff.
What You'll Do:
Assess residents' clinical needs and respond proactively Support and monitor individualized care plan implementation Provide leadership and guidance for our nursing staff Promote a culture of accountability, teamwork, and quality outcomes Ensure compliance with state and federal regulations Work closely and collaborate with the DON and ADON to maintain consistency of workflow within the unit Ensure compliance with state, federal, and company regulations Monitor documentation, physician orders, labs, and care coordination while supervising daily clinical operations Support staffing, performance oversight, and quality improvement efforts Serve as a liaison between nursing staff, physicians, residents, and families Participate in audits, care plan meetings, infection control, and safety initiatives Take part in the administrative on-call rotation as assigned Why You'll Love Working with

What We're Looking For:
As a condition of employment, nurses must meet Primary State of Residence (PSOR) and Nurse Licensure Compact (NLC) requirements. Nurses must maintain an active nursing license in their Primary State of Residence. Nurses whose Primary State of Residence is South Carolina must hold a South Carolina nursing license and a South Carolina driver's license or state-issued ID with a South Carolina address. Candidates residing outside of South Carolina must maintain appropriate licensure in accordance with their Primary State of Residence and NLC requirements. Current South Carolina RN license in good standing. Strong clinical assessment and critical thinking skills Leadership experience or a passion for mentoring others Excellent communication and organizational skills Commitment to delivering compassionate, person-centered care Weekend availability and flexibility Long-term care experience preferred but not required Ready to Join Us?
