Company Overview No Place Like Home, Inc. is a pediatric home health agency dedicated to providing compassionate and high-quality nursing care to children in Fayette, Shelby, and Tipton County. We are seeking an experienced and compassionate Registered Nurse (RN) Supervisor to lead and support our nursing team while ensuring the highest quality patient care in the home health setting. This is an excellent opportunity for a clinically strong nurse leader who is passionate about patient outcomes, staff development, and regulatory compliance. As an RN Supervisor, you will collaborate with patients, families, physicians, case managers, and interdisciplinary team members to coordinate care, oversee clinical operations, and support field nursing staff. Job Overview Develop, implement, and evaluate individualized patient care plans in accordance with physician orders. Conduct patient assessments and oversee admissions and start-of-care processes. Supervise and support RNs, LPNs, and CNAs in the field. Monitor quality of care through chart audits, documentation reviews, and clinical oversight. Ensure compliance with physician orders, regulatory requirements, and agency policies. Assist with hospital transfers, resumptions of care, and care coordination following discharge. Investigate and resolve patient, family, and staff concerns. Participate in staff orientation, competency evaluations, and ongoing clinical education. Review medication records, treatment documentation, and controlled substance records for accuracy and completeness. Collect and manage required clinical data and assessments, including OASIS documentation. Remains accessible via phone outside of regular business hours for emergencies that can only be addressed by the Clinical Management team. Qualifications Required Graduate of an accredited nursing program. Current Registered Nurse (RN) license in good standing. Minimum of two years' experience as a licensed RN. Strong clinical assessment and care planning skills. Ability to work independently while managing multiple priorities. Must be willing to travel to patient homes located throughout Fayette, Shelby, and Tipton counties. Excellent communication, leadership, and organizational skills. Preferred Previous supervisory or management experience. Home health, hospice, or community-based nursing experience. Pediatric experience. Experience with OASIS documentation and electronic medical records. Physical Requirements Ability to lift and carry up to 50 pounds. Ability to push, pull, bend, stoop, reach, and assist with patient handling as needed. Ability to travel to patient homes and other care settings. Ability to work in environments with potential exposure to bloodborne pathogens and communicable diseases.
