Job Description

CareRite Centers seeks a Night Shift LPN/RN Supervisor to lead clinical care in our skilled nursing and rehabilitation center. In this resident-centered role, you will oversee night-shift nursing and CNA teams, coordinate and monitor post-acute and long-term care, and ensure adherence to care plans, policies, and regulatory standards. Responsibilities include medication administration oversight, clinical documentation, rounds, incident management, and communication with families and on-call providers. You will mentor staff, support a compassionate, hospitality-driven environment, and contribute to quality, safety, and performance improvement initiatives. Responsibilities Supervise night-shift LPNs, RNs, and CNAs to ensure safe, high-quality resident care Oversee medication administration, treatments, and physician orders for post-acute and long-term residents Conduct rounds to assess resident status, respond to changes in condition, and coordinate appropriate interventions Ensure accurate, timely clinical documentation and compliance with regulatory and facility standards Serve as the on-site clinical lead during night shift, handling emergencies, incidents, and family concerns Collaborate with interdisciplinary team members to support individualized care plans and quality outcomes Coach, mentor, and support staff performance, reinforcing resident-centered and hospitality-driven care Monitor infection control, safety practices, and risk management throughout the unit Communicate effectively with on-call providers, day leadership, and families regarding resident needs Contribute to quality improvement and performance initiatives focused on resident experience and outcomes.
