Charge Nurse - Long Term Care
Job Description
Overview Join our healthcare team as a Charge Nurse (RN or LPN), where you will lead and coordinate patient care in a patient-centered environment. This pivotal role empowers you to oversee nursing assistants, and ensure high-quality clinical practices. As a Charge Nurse, you will serve as a vital link between management and frontline staff, promoting efficient operations while maintaining compliance with healthcare standards. (NOC shift LPN will receive a $3 shift differential.) Responsibilities Supervise and support nursing staff during shifts, ensuring adherence to clinical protocols and safety standards. Coordinate patient care activities, including assessments, medication administration, IV infusions, and documentation using EMR systems like MatrixCare. Oversee patient assessments, vital signs, and physical examinations to develop individualized care plans. Ensure compliance with HIPAA regulations and maintain accurate medical records and documentation for all patient interactions. Requirements Valid RN or LPN license with current state licensure required; additional certifications such as Basic Life Support (BLS) or Cardiopulmonary Resusitation (CPR) required. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or MatrixCare for accurate medical documentation. Strong knowledge of anatomy, physiology, medical terminology, and diagnostic evaluation techniques. Experience managing cases involving hospice & palliative medicine or behavioral health is highly desirable. Compassionate approach to working with individuals across age groups including geriatrics, or patients with developmental disabilities or dementia care needs.
