Job Description

CareRite Centers seeks a Night Shift LPN/RN Supervisor to lead clinical care for residents in a skilled nursing and rehabilitation setting. In this onsite role, you will oversee nursing staff, coordinate and prioritize care, and ensure adherence to care plans, infection control, and regulatory standards. Responsibilities include medication administration, assessments, incident management, and directing CNAs and nurses to maintain safety and quality outcomes. You will collaborate with interdisciplinary teams, support family communication, mentor staff, and contribute to a resident-centered, compassionate, and hospitality-driven environment focused on continuous improvement. Responsibilities Supervise LPNs, RNs, and CNAs during night shift to ensure safe, high-quality resident care Conduct clinical assessments, monitor resident conditions, and respond to changes in status Administer medications and treatments in accordance with provider orders and facility policies Ensure accurate, timely documentation and EMR charting of care, incidents, and assessments Oversee staffing, assignment coordination, and workflow for night-shift nursing team Uphold infection control, safety, and regulatory standards in a skilled nursing environment Collaborate with interdisciplinary team members and communicate with families as needed Lead and mentor nursing staff, providing guidance, coaching, and support for best practices Participate in handoff reports, care plan updates, and quality improvement initiatives Address and report incidents, falls, and changes in condition, ensuring appropriate follow-up Required Skills Medication administration Wound care management Care planning and documentation (EMR) Clinical assessment and triage Infection control practices Supervisory and staff scheduling skills Regulatory and compliance knowledge (LTC regulations) Fall and risk management IV therapy and treatments Report writing and handoff communication
