We are seeking a dynamic and compassionate Licensed Practical Nurse (LPN) to step into the vital role of Charge Nurse within our healthcare team. In this energetic position, you will lead nursing staff, coordinate patient care, and ensure the delivery of high-quality services across various clinical settings. Your expertise will help foster a positive, safe, and efficient environment where patient well-being is prioritized. This role offers an exciting opportunity to utilize your extensive nursing skills, including critical care experience, medical documentation proficiency, and leadership capabilities, to make a meaningful difference in our residents' lives. Duties Assume leadership responsibilities by supervising nursing staff and delegating tasks effectively to ensure seamless patient care delivery. Oversee daily clinical operations, ensuring compliance with long-term care regulations, HIPAA confidentiality policies, and safety standards. Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication administration, utilizing EMR/EHR systems for accurate documentation. Manage complex care needs such as IV infusions, catheterizations, wound care, pain management, and dementia or Alzheimer's care. Coordinate interdisciplinary communication for discharge planning, case management, and family updates while maintaining high standards of patient safety and satisfaction. Ensure adherence to infection control protocols and participate in quality improvement initiatives to enhance care outcomes. Provide training and mentorship to nursing staff on best practices in geriatrics, trauma response (Level I & II trauma center experience), emergency medicine procedures, and other specialized areas. Experience Valid Licensed Practical Nurse (LPN) license with current state certification. Proven leadership experience as a Charge Nurse or in a supervisory nursing role within long-term care facilities or hospital settings. Extensive critical care experience including ICU or emergency room environments preferred. Familiarity with EMR/EHR systems such as Cerner or Athenahealth for accurate medical documentation. Strong background in managing complex patient needs involving
ICD-9/10
coding, medication administration, tube feeding, ventilator management, and infection control practices. Experience working with diverse populations including geriatric patients, individuals with developmental disabilities, and those requiring memory or dementia care. Knowledge of long-term care regulations and skilled nursing facility standards is highly desirable. Additional experience in hospice care, acute pain management, trauma response (Level I & II Trauma Center), pediatric or neonatal ventilator management is a plus.
