Job Description

Overview Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN) - Charge Nurse, where your expertise will lead compassionate patient care and operational excellence. In this vital role, you will oversee daily nursing activities, coordinate patient assessments, and ensure high standards of clinical practice across diverse care settings. We are seeking energetic, detail-oriented professionals committed to delivering exceptional nursing services in a fast-paced, team-oriented environment. Responsibilities Provide comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews using EMR (Electronic Medical Records) systems such as Epic or Cerner. Supervise and coordinate nursing staff to ensure efficient delivery of care aligned with HIPAA regulations and clinical protocols. Administer medications, injections, IV infusions, and perform catheterizations with precision and adherence to safety standards. Manage complex cases involving acute pain management, dementia care, Alzheimer's care, and memory care while collaborating with interdisciplinary teams. Lead triage activities in urgent or emergency situations, utilizing critical thinking skills to prioritize patient needs based on severity and acuity levels typical of Level I Trauma Centers or ICU environments. Ensure accurate documentation of medical records, medical terminology entries, CPT coding for procedures like suturing or spinal taps, and
ICD-10/ICD-9
coding for diagnoses. Support discharge planning and post-acute care coordination to facilitate smooth transitions for patients returning home or to long-term care facilities. Maintain infection control standards through aseptic techniques and sterile processing procedures during specimen collection or medical imaging procedures. Provide education on medication administration, nutrition, wound care, and disease management tailored to diverse populations including pediatrics, geriatrics, neonatal ventilator patients, or individuals with disabilities. Experience Valid LVN license with current state licensure required; additional certifications in Basic Life Support (BLS), IV insertion/injection administration preferred. Demonstrated experience working in various healthcare settings such as hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, outpatient clinics, or home & community care environments. Proven track record in managing complex patient cases involving infectious disease care, hospice services, or behavioral health interventions. Prior experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks is highly desirable. Familiarity with advanced clinical procedures including ventilator management, tube feeding protocols, cardiac catheterization support, or spinal taps is advantageous. Strong knowledge of physiology and anatomy principles combined with excellent patient assessment skills across diverse age groups—from neonatal to geriatric populations. Ability to work effectively under pressure in fast-paced environments such as emergency rooms (ER), ICU units (including PICU), or surgical settings requiring aseptic techniques and sterile processing expertise.
