Job Description
Touchstone Communities seeks an LVN/RN Charge Nurse for overnight shifts in our skilled nursing and rehabilitation community. In this leadership role, you'll provide hands-on clinical care, oversee shift operations, and support CNAs while ensuring resident safety and dignity. Responsibilities include medication administration, wound care, assessments, documentation, and coordination with physicians and families. You'll model servant leadership and person-centered care within a faith-based, team-focused culture, with opportunities to grow clinical and leadership skills while impacting residents' recovery and quality of life. Responsibilities Provide overnight skilled nursing care to residents in a skilled nursing/rehab setting Serve as charge nurse overseeing LVN/RN workflows and ensuring safe staffing and shift handoffs Administer medications, treatments, and monitor resident conditions per care plans and physician orders Lead and mentor nursing and CNA teams, promoting servant leadership and teamwork Document assessments, interventions, and outcomes accurately in the medical record Communicate with residents, families, and interdisciplinary team regarding care changes and goals Respond to emergencies, perform clinical interventions, and escalate concerns promptly Support rehabilitation and long-term care goals, promoting dignity, comfort, and person-centered care Required Skills Medication administration Wound care management Skilled nursing/long-term care Charge nurse leadership Care planning and coordination Electronic health records (EHR) documentation Assessment and vital signs monitoring IV therapy and injections Infection prevention and control Emergency response and triage
