Responsibilities include:

Provide direct nursing care to Assisted Living and Memory Care residents Administer medications in accordance with physician orders, state regulations, and community policies Complete resident assessments and monitor changes in condition Maintain accurate documentation and resident medical records Develop and update individualized care plans Communicate with physicians, families, and interdisciplinary team members Lead, mentor, and support caregivers during assigned shifts Assist with training and coaching new team members Ensure compliance with state regulations and community policies Promote a positive, resident-centered culture focused on exceptional care Perform other duties as assigned Qualifications Current Texas LVN/LPN License required Current CPR Certification required High School Diploma or GED required Minimum one (1) year of nursing experience required Assisted Living, Memory Care, Long-Term Care, Skilled Nursing, or Rehabilitation experience preferred Previous Charge Nurse or leadership experience preferred Excellent communication, organizational, and leadership skills Ability to make sound clinical decisions while working independently Ability to stand, walk, bend, lift up to 25 pounds unassisted, and assist residents with mobility Schedule Full-Time Weekends and holidays as needed Apply Today!
