Responsibilities Supervise and coordinate nursing staff to ensure seamless delivery of patient care across all shifts. Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication administration. Manage complex clinical situations such as acute pain management, IV infusion therapy, airway management, and catheterization procedures. Oversee medication administration in compliance with HIPAA regulations and safety protocols. Utilize electronic health record (EHR) systems to document medical records accurately and efficiently. Ensure infection control practices are followed rigorously to maintain a sterile environment and prevent healthcare-associated infections. Collaborate with interdisciplinary teams on diagnostic evaluation, case management, and discharge planning for post-acute or long-term care patients. Maintain compliance with long-term care regulations and documentation standards for skilled nursing facilities or assisted living environments. Experience Valid LVN license with current state licensure required; additional certifications in CPR/BLS preferred. Demonstrated care experience within LTC settings Proven background working with diverse populations including geriatrics or individuals with developmental disabilities. Familiarity with ICD coding (ICD-10), medical terminology, and electronic health records Prior roles involving hospice care, infectious disease management, or acute pain management are a plus. Knowledge of long-term care regulations and experience working in skilled nursing facilities or assisted living communities is beneficial. Strong communication skills with the ability to educate patients and caregivers effectively while maintaining clinical confidentiality policies.
