Job Description Summary:
The Care Manager works in partnership with patients, families, caregivers and providers to deliver targeted care that is organized, coordinated, cost effective and patient-centered. The Care Manager collaborates with interdisciplinary staff and stakeholders within the care continuum to promote patients' optimal health through disease prevention and management strategies. Evidence-based interventions are applied to populations for cost efficient and outcome-based care. Outcomes include disease-based quality measures, patient satisfaction metrics, access to services, and hospital/ED utilization rates.Responsibilities And Duties:
