Job Description

Sevita is seeking a Direct Care Supervisor to lead teams delivering home and community-based services for individuals of all ages and abilities. In this role, you will oversee direct support staff, ensure person-centered plans are followed, and maintain high standards of safety, quality, and compliance. You will coach, schedule, and develop staff; coordinate with families and clinicians; and provide hands-on support as needed. Responsibilities Supervise direct care staff providing home and community-based services Ensure person-centered support plans are implemented and followed Monitor quality of care, safety, and compliance with policies and regulations Coach, train, and schedule staff to meet individual care needs Coordinate with families, clinicians, and community partners Document services, progress, and incidents accurately and timely Support individuals with daily living, behavioral, and health needs Model compassionate, trauma-informed, and respectful care Participate in hiring, performance feedback, and staff development Respond to emergencies and escalate concerns appropriately Required Skills Care coordination Staff supervision Behavioral support implementation Person-centered planning Electronic health/care documentation Training and coaching Crisis de-escalation Regulatory and safety compliance Time and schedule management Communication with families and clinicians
