LVN License in good standing with the state Ability to relate positively to residents and families Capable of maintaining regular attendance. Must be able to supervise and work cooperatively with other employees. Meet all local health regulations, and successfully complete a post-offer health assessment. Physical and sensory requirements (with or without the aid of mechanical devices): walking, reaching, climbing, bending, lifting, grasping, fine hand coordination, pushing and pulling, ability to hear and respond to pages; ability to read and write; ability to communicate both verbally and in writing with residents, families, personnel, vendors and consultants; ability to remain calm under stress; ability to distinguish smells, tastes and temperatures. Must be capable of performing the essential functions of this job, with or without reasonable accomodation.Responsibilities:
Under the supervision of the Director of Nursing (DON), the charge Nurse Provides nursing Care to residents as prescribed by the physician and in accordance with standards of nursing practices and regulations and supervises the Certified Nursing Assistant (CNA) Staff in the delivery of direct resident care. The LVN Charge Nurse reports to the Assistant Director of Nursing (ADON) and the Director of Nursing (DON).Key Responsibilities:
Prepare and review medical records and nurse's notes and assess residents to evaluate their physical and emotional status; monitor resident activity and provide nursing care according to the physician's orders, care plans, established standards, and facility policies. Coordinate and assist or prepare for new admissions, transfers, and discharge of residents. Transcribe physicians' telephone orders. Communicate with residents, family members, other interdisciplinary team members and management regarding resident status. Prepare and administer medications as ordered by the physician in accordance with nursing standards and facility policies. Sign, date, and perform all charting and record keeping in accordance with established policies and procedures. Supervise and direct the CNA staff in the delivery of resident care and monitor their performance. Schedule CNA job assignments, set priorities, assist, coach and counsel as necessary. Understand, comply with, and promote all rules regarding residents' rights Assist and participate in developing and implementing a written care plan for each individual resident that addresses the needs of the resident. participate in Inservice training; assist in implementing orientation programs and job skills training; maintain professional competence. monitor environment and care practices of nursing personnel to ensure compliance with established safety, fire, disaster, infecion control, and all other departmental policies and procedures; ensure cleanliness and safety of work and treatment areas. other duties as assigned.
