In this pivotal role, you will oversee clinical operations, manage nursing staff, and coordinate comprehensive home health services. Your leadership will drive quality improvement initiatives, compliance with healthcare regulations, and foster a compassionate environment that prioritizes patient safety and satisfaction. This is an exciting opportunity for a motivated nursing professional eager to make a meaningful impact in community-based care. Responsibilities Lead and supervise the home health nursing team, including hiring, training, evaluating performance, and fostering professional development. Develop and implement clinical protocols aligned with state healthcare regulations and industry standards such as NCQA and HIPAA. Oversee patient assessments, care planning, discharge planning, supervisory visits and ongoing case management to ensure optimal health outcomes. Coordinate interdisciplinary collaboration among physicians, therapists, social workers, and other healthcare providers to deliver holistic patient care. Ensure compliance with documentation standards using EMR (Electronic Medical Records) systems Wellsky ( Kinnser); maintain accurate medical records and medical documentation. Monitor quality metrics related to patient care, safety protocols such as infection control and aseptic techniques, and manage incident reports or adverse events. Manage operational aspects including staffing schedules, resource allocation, budget adherence, and ensuring nursing policy compliance. Requirements Valid RN (Registered Nurse) license with current state licensure. Extensive experience in home health nursing or related settings such as medical-surgical, geriatrics, or behavioral health. Proven leadership skills with prior nurse management or supervisory experience in a healthcare environment. Critical care experience including ICU or emergency medicine is highly desirable; familiarity with Level I & II trauma center protocols is a plus. Proficiency in EMR/EHR systems such as Wellsky ( Kinnser); strong medical documentation skills. Knowledge of healthcare regulations including HIPAA, long-term care regulations, and Medicare/Medicaid policies. Competence in clinical procedures like IV infusion, catheter care, medication administration, tube feeding, phlebotomy, venipuncture, and sterile processing techniques. Strong communication skills for effective managerial communication and team collaboration; ability to handle complex case management scenarios. Additional qualifications include experience working with individuals with disabilities or developmental delays; familiarity with ICD coding (ICD-9/ICD-10), utilization review/management processes; knowledge of dementia care and memory support strategies.
