Who We're Looking For We are looking for an RN who combines strong clinical judgment with leadership and compassion. The right candidate is comfortable assessing clients in their homes, developing Care Plans, supporting and educating caregivers, collaborating with the office team, and helping us maintain a high standard of care as we grow. Key Responsibilities
  • Complete initial client assessments and reassessments in clients' homes; develop, review, and update individualized Plans of Care.
  • Provide clinical oversight, case management, field supervisory visits, and authorized skilled nursing visits as needed.
  • Educate and coach caregivers; create and participate in clinical caregiver training during quarterly caregiver meetings.
  • Collaborate with the Branch Manager to lead quarterly Quality Assurance (QA) meetings and review clinical and operational quality trends.
  • Partner with the Client Care Coordinator during new-client starts. The RN completes the clinical assessment and Plan of Care, while the Client Care Coordinator primarily completes service agreements and other service/admission paperwork.
  • Serve as a backup admissions team member when needed, which may include completing both the clinical assessment and admission paperwork for assigned cases.
  • Maintain timely clinical documentation in our electronic filing system, coordinate with physicians and other healthcare professionals, and participate in on-call/cross-coverage responsibilities as assigned. Qualifications
  • Current, unrestricted Virginia Registered Nurse (RN) license required.
  • At least 1 year of nursing experience in home care, home health, hospice, hospital, private duty nursing, or a related setting preferred.
  • Home care leadership, case management, supervisory, or Director of Nursing experience preferred.
  • Strong clinical assessment, care-planning, communication, leadership, and organizational skills; valid driver's license, automobile insurance, and reliable transportation required. Compensation & Benefits
