Job Description

We are looking for a Resident Care Manager to coordinate holistic and personalized care. You will be responsible for facilitating resident and family education, communicating with providers, and designing a care plan that addresses individual resident needs. Forming care plans and driving their implementation. The ideal candidate will be a responsible and well-trained professional able to motivate and lead others to provide the best care. You need be able to implement best practices with the goal of promoting each resident's well-being through high quality nursing care. Responsibilities Monitor resident's condition and assess their needs to provide the best possible care Assess symptoms and communicate any changes to the provider and the team Collaborate with the department head team to devise individualized care plans for residents Foster a supportive and compassionate environment to care for residents and their families Coordinate the assessment process including admission/ discharge/quarterly assessments, consents, order processing, and SBAR communication Review medication regimes for accuracy and regulatory compliance Process orders and initiate appropriate alerts Maintain and read the 24 hour report Rounding every two hours assessing nursing, NAC staff, and providing role model leadership Skills Long-term care experience Proven experience as a licensed nurse Excellent knowledge of nursing care methods and procedures A team player with excellent communication and interpersonal skills Responsible and compassionate Outstanding organizational and multi-tasking skills Excellent problem-solving skills Valid nursing license
