HCBS Executive Director Position Summary St. Camillus is seeking a compassionate and experienced Wellness Nurse Manager/Director to support the Wellness Program and Private Duty Services for our Independent Living residents. This role oversees day to day wellness operations, provides clinical support through a Wellness Clinic, delivers resident education, and responds to health related concerns and emergencies. The ideal candidate is a collaborative nursing professional who is comfortable working independently, supporting change, and partnering closely with Private Duty and campus leadership to promote resident wellness and safety. Key Responsibilities Staff the Independent Living Wellness Center during designated hours for resident consultations, blood pressure checks, and health education Plan, support, and implement wellness and educational programs for residents Participate in pre admission evaluations to support smooth resident transitions Develop and update plans of care for Private Duty clients Conduct supervisory visits for Private Duty caregivers and support caregiver onboarding and training Collaborate closely with the Private Duty Manager to meet resident needs Manage infection control efforts, including outbreak management, in coordination with campus Infection Control and Occupational Health Respond calmly and appropriately to emergency situations and support emergency preparedness planning Coordinate resident and family meetings related to changes in level of independence; document and communicate concerns as needed Support discharge planning and assist residents transitioning to other levels of care on campus Maintain confidentiality and adhere to all St. Camillus policies, procedures, and code of conduct Perform other duties as assigned Qualifications Active Wisconsin Nursing License (LPN preferred) Minimum 2 years of nursing experience, preferably in a senior living or community based setting Strong interpersonal, communication, and organizational skills Ability to work independently and collaboratively in a team environment Proficiency with Microsoft Office (Word, Excel, Outlook) and electronic medical records Strong problem solving, conflict management, and decision making skills Ability to respond effectively in emergency situations Ability to safely perform essential job functions with or without reasonable accommodation Why Join St. Camillus?
