We're looking for a caring, organized, and relationship driven Registered Nurse (RN) to join our team as a Client Care Advisor . As Home Instead continues to grow throughout North Central West Virginia , we're seeking an RN with experience in the West Virginia Medicaid Aged and Disabled Waiver (ADW) and Home and Community-Based Services (HCBS) programs. In this role, you'll help oversee participant care, develop individualized plans of care, train and support staff, and ensure compliance with state program requirements.
What You'll Do:
Conduct in home care consultations and client assessments. Develop individualized plans of care that promote safety, independence, and quality of life. Coordinate services and collaborate with clients, families, Care Professionals, and community partners. Train and support staff on ADW and HCBS program requirements and best practices. Complete quality assurance visits to ensure exceptional care and client satisfaction. Monitor participant progress and maintain accurate documentation. Serve as a compassionate advocate and trusted advisor for clients and their families.
What We're Looking For :
Active West Virginia Registered Nurse (RN) license. Experience with the West Virginia Medicaid ADW and HCBS programs is required. Previous experience in home care, home health, hospice, case management, or healthcare sales is preferred. Experience working with older adults. Excellent communication, interpersonal, and organizational skills. Ability to travel throughout our service area for client visits. Flexibility to work occasional evenings and weekends as needed.
